Appendix B - Policy Agreement Form

Appendix B is the standard policy agreement form which all compliant companies are required to sign before being accepted on to the list of recognised companies.

Please click on the link below
When the new window opens select the "File" menu
Click on "Save As"
Change the "Save as Type" option from "Word Document" to "Word Template"
Give the document a suitable name and click on "Save"
It will be saved the default location for your "Word" templates
